Comprehensive Wellness Membership
A simple, affordable way to access acute care, chronic care, wellness support, and hydration therapy. Some labs and additional testing are not included. IV Hydration and IM Vitamins are in-home visits billed an additional $62.50 for any subsequent visits not covered by your membership, except for annual physical exams. Virtual visits are billed an additional $45 for any subsequent visits not covered by your membership.

No long-term contracts; cancel anytime with 30-day notice
Membership fees do not replace insurance

📄 Membership Disclosures & Important Terms
Membership Continuity & Re-Enrollment
If a member cancels their plan or fails to pay the monthly subscription fee, TruGrace Medical Clinic, PLLC cannot guarantee the same pricing, discounts, or benefits if they choose to re-enroll in the future. Membership rates and benefits are subject to change at any time.
Grace Period for Missed Payments
Members have a 7-day grace period from the scheduled due date to submit any missed membership payment.
If payment is not received within this timeframe, the membership will be automatically suspended.
Services, discounts, and benefits will not be available during the suspension period.
No Rollover of Benefits
Membership benefits do not roll over.
All included visits, discounts, and services must be used within the same monthly billing cycle.
Unused benefits expire at the end of each cycle and cannot be carried forward, credited, or refunded.
Service Access During Delinquency or Cancellation
If a membership becomes inactive due to non-payment or cancellation:
Access to member pricing, discounts, and services included will stop immediately.
Standard visit rates and service fees will apply.
Reinstatement will require enrollment under the current membership pricing and terms at that time.
